Assessment Information Provided by Physicians or Other Professionals
As we continue to ensure a safe and supportive learning environment for all our students, we kindly ask that you submit any updated medical assessments for your child to the school office. These assessments provided by your physician or other professionals help everyone better understand and accommodate your child's needs, ensuring they receive the best possible support in their education. This information will be returned to the professional in one response to support communication between both parties.
Examples of these assessments are, but are not limited to: BASC, Conners: Comprehensive Behavior Rating Scale, CAP: Child Attention Profile, and the Vanderbilt.
You can submit the documents by:
- Dropping them off at the school offices - Tiffany Creek Elementary or Middle/High School Office
- Faxing them to 1-715-643-3127 and attention to Rebecca Hanestad (Special Education/504 Coordinator) or the school principals Mr. Gretzlock (MS/HS) or Mr. DesJarlais (TCE)
